Tournament Registration

This is the "Registration Section" of the OSU Women's Club Ice Hockey Team Ice Breaker Tournament web site.  There are 3 steps necessary to register for the Ice Breaker tournament:

Step 1 is to fill out the form below completely and click Submit at the bottom.

Step 2 is to completely reserve your teams rooms at a participating Marriott hotel.*  Your team will be required to submit your list of Marriott rooms and confirmation numbers upon physical registration,  Sept 12th.

Step 3 is payment information.  Payment can be submitted via personal check.  A $50 deposit is required to secure your team's spot.  Space is limited - 1st come, 1st serve.  Mail all checks to: Ohio State University Womens Ice Hockey Club, INC., 1777 Mckinley Ave., Columbus, OH  43222
